Community Resources and Care Coordination

Seven Hills Foundation & Affiliates offers accessible community resources and care coordination services that help connect individuals, children, and families with access to information, referrals, and wrap-around supports to address all of their personal and health related goals. Our Behavioral Health Centers work in an integrated fashion with our Family Resource Centers, Family Support Centers, and Care Coordination Services to streamline the intake and assessment process so that consumers are connected to all of the services and support they need to Be Well.

Family Resource Centers

We are part of several statewide networks of community-based providers offering a variety of services such as multi-cultural parenting programs, support groups, access to educational and social supports, information and referral resources, and care planning.

Community Connection Coalitions provide safe and neutral settings for residents and service providers, including DCF, to communicate, make decisions, share resources, and coordinate action planning.
