Person-Centered, Individualized Community Supports

Seven Hills' supportive living program is customized for people living in their own apartments or homes who require up to 40 hours of staffing support per week for DDS eligible adults. This person-centered service is intended to support people to live as independently as possible to pursue their dreams and experience the joys and challenges of everyday life. 
 
Supports are flexible in response to the growing and changing demands of each person’s interests and well-being. Seven Hills responds to the changing needs of individuals by adapting the schedules of staff within the contracted guidelines to enable the individuals to enjoy community events, attend medical appointments, and manage their needs. 
 
Services may include assistance with menu planning, financial management, health care planning, adult companionship, leisure time planning, PCA (Personal Care Attendant) surrogacy, representative payee, and management of benefits. Seven Hills also accepts private pay arrangements for these services and is committed to working with individuals and families to craft the model that works best for their needs.

Who is Eligible for Supportive Living?

Supportive living programs are designed for individuals who have intellectual, or developmental disabilities, and may have physical disabilities as well. These programs are available to individuals aged 22 and older.
