Seven Hills Foundation is proud to announce that it has been awarded a contract by the Massachusetts Department of Developmental Services (DDS) to implement a Home Based Residential Supports Pilot Program in Central West Massachusetts. This new and innovative alternative approach to 24/7 residential services will provide 4-5 individuals who are supported by their loved ones in the family home with an integrated array and coordinated package and schedule of support services that provide an alternative to a more restrictive residential placement. Integral to the Pilot is the active engagement of the people who support the individual in ways that augment paid staff services and support community membership and relationships. Navigation, Services and Supports, and Flexible Funding are major service components of this new model. Individuals are referred by DDS and must have priority 1 status. Seven Hills is excited to work with these individuals and families using a family-centered approach to helping them achieve their individualized goals and objectives.
